Fixes for needless refetching of weakly reintegrated files, and a bad optimization error in log-based resolution which occasionally cancelled out the wrong operations, distribution of rpc2 and rvm as separate packages, removal of a bunch of unused code, hierarchical file cache for Coda clients, lazy creation of hierachical directory trees, correct handling of writeback caching on replicated volumes, and filenames in repair files sometimes got truncated.
Features: multiple servers on the same machine, cross-directory rename resolution.
Fixes: token file decoding, bugs that led to bitmap corruption in some setups, server crash on some repairs, volume creation on new servers, repair tool to allow spaces in filenames, codacon connecting to bogus address, libal/libdb2 compatibility.
